Dear friend, although I am sorry that I'm unable to answer most of your questions, I can tell you that my brother didn't have any recommendations from a doctor when he went to The Anderson Cancer Center in Houston, TX. for a second opinion.was diagnoised with prostate cancer. He went on the internet to get the info. that he needed, called down there and they set everything up for him. You can type in Anderson Cancer Center on the computor, and it will bring up all kinds of info.
My brothers insusrance paid for the medical procedures there, however he is not on medicare yet, but I presume that medicare would do the same, but it would be wise to check with your insurance carrier as well as the Anderson Cancer Center. Unfortunately Insurance. at least his does not pay for transportation there and back, or for hotel, food etc. Sorry I couldn't be of more help, but hope that this info. will help alittle.
