My husband is also on a lower dose. Right now he's at 300mg. During radiation he was on 160mg. The first dose of the 5/23 cycle they upped it to 400mg. He had a bad reaction starting about day 5-6; got an itchy rash all over his body, was extremely weak, etc. It was pretty bad. The onc. isn't certain it was the Temodar since he had been on it before although the timing certainly worked out that way. He was also taking Keppra and a cholesterol med at the time. He stopped both of these, he hadn't ever had a seizure and they aren't real worried about cholesterol right now so that made stopping these easy.
We were going to switch to Avastin/CPT-11, the onc. didn't want to 'challenge his body like that again', but his MRI results after that first round of Temodar were so good the onc. wanted to stick with it, but said we'd adjust the dose and find a level that worked.
The next round he did 150mg, then 250mg, now he's on his second round of 300mg. Not sure if this is where we're staying for the duration or whether they'll bump it up again (of course my husband cringes at the the thought of a higher does).
His blood counts have remained good throughout thought. It's still a bit of a mystery where that reaction came from. The good news is that it's still working, the MRI's continue to be good.
**husband dx 8/07, GBM, surgery, IMRT radiation/Temodar, currently on 5/23 Temodar, MRI's good.
