Subject: HELP got medical records don't understand!!
Date: 04/15/2008

I have posted on board before but was asked questions could not answer. I am ashamed to say I finally got medical records, should have requested sooner but didn't. I understand some but, asome I do not this is a timeline of my medical rollercoaster ride:

Sept. (06) Pap test: Atypical squamous cells of undetermined significance  (ASC-US) HPV positive

Oct. (06) colposcopy and ECC: ECC reveiled squamous cells with degenerative change, but no definitive dysplasia or malignancy seen.        Biopsy reveiled moderate to severe dysplasia Dr said CIN II -CIN III.

Nov. 06) Cone biopsy using LEEP: Junctional mucosa with koilocytic-like cells, suggestive CIN I. All surgical margins free of lesion.  No similar CIN II lesion is found in this specimen.

I bled for a few months did not go again till March and got pap again.

March(07) pap test: Atypical squamous cells can not exclude high grade SIL (ASC-H) HPV negative.

May(07)pap test: Atypical squamous cells of undetermined significance     (ASC-US) HPV negative.

Aug(07)pap test: Negative for intraepithelial lesion or malignancy. Reactive cellular changes(don't understand)

Feb(08)pap test: Rare atypical squamous cells of undetermined significance (ASC-US) HPV positive (don't understand)

Ok first did not know my tests were coming back atypical which to my knowledge can mean abnormal so why not more testing been done, and I don't understand Feb diagnosis Rare? does it mean what it says or does it mean medical terms few? very confused. This all was word for word on my reports need help anyone have the same or very close. My next appt. is not till August freaking a little and not sure if thats too long to wait since all pap tests may have been coming out abnormal since Cone Biopsy using LEEP.

Aug(07)pap test: Negative for intraepithelial lesion or malignancy. Reactive cellular changes - The purpose of the Pap smear is to screen for the presence of malignancy or pre-malignant conditions, the absence of these is considered normal.

Feb(08)pap test: Rare atypical squamous cells of undetermined significance (ASC-US) HPV positive- This means that your situation has not turned into dysplasia yet.  Most of the time (70-85%) ASCUS Pap smears revert to normal on their own over time. So I am assuming your doctor is going to continue to monitor you.  This should not get you majorly concerned.  There are several tell-tail signs you have to progress to in your pap to get to cervical cancer and right now you haven't even progressed to dysplasia yet.

I kinda understand what ur saying only problem is my very first pap said same thing and ended up after colposcopy done was CIN II-CIN III so, what I'm worried about is same thing coming up. I also spoke with someone on the board before who suggested all was possibly not removed no fault to dr. First pap test reveiled same Atypical squamous cells with undetermined significance.

Mommy4 is right. Atypical means cellular changes, which is why your doctor wants to monitor you. Here are some links that migh help explain this for you. Some of them are older, but the info is still helpful.

I definitely understand your concern about it turning into CIN II or III, again, though. I'm glad you got your lab reports. But has your doctor explained any of this to you or given you any information to read? In addition, have you had the HPV DNA test done, yet, to find out what strain(s) you have? There is currently Digene's test. But Roche also has a new HPV DNA test, too, which I have been told is even more accurate. I had that done. It's just harder to get ahold of the new test because insurance companies may not want to pay for it. You may have to ask for it, and if your doctor doesn't have it, check a teaching hospital about it. If you can find out what strain(s) you have, it could definitely help your doctor and you decide how to address this. It made all the difference for me. In addition, if you're not comfortable with the answers you're getting from your doctor or if you don't feel you're being given all the information you need, please don't hesitate to get a new one. In any case, a second opinion always helps.
