Can ultrasound scan find Stage One ovarian cancer?

Hi - does anyone know if ultrasound scans - whether internal or surface - can detect ovarian cancer at Stage One? Or, indeed, Stage Two.

Has anyone here been DX stage one (or two), and if so, how did it happen?

Many thanks, Julie

Hi,

I was diagnosed with stage 1 ovarian cancer one year ago.  I initially had a transvaginal ultrasound and an MRI followed by hysterectomy and chemo.  I was informed that a definate diagnosis cannot be made until surgery.  I went to a gynecological oncologist from the start.  That is something you should seriously consider if you are not already doing so. My "regular" gynocologist informed me that she could do the surgery, and if it was cancer, there would be other doctors standing by to assist as needed depending on what was found.  She also explained to me the benefits of going to a gynecological oncologist from the start  and recommended someone she had total confidence in and for that I will always be greatful. Most importantly, stay positive and take one step at a time.  Today I am healthy  and give thanks for each new day.  Live Strong.  Pati

Thank you so much for your positive and encouraging reply. I appreciate that no screening method is 100% effective, but in the UK there seems to be official 'uncertainty' as to whether US scans can indeed detect as early as stage one. We currently have a ten year trial running (reporting in two years ) of something like 200,000 women, to provide the evidence, they hope, either for or against introducing a nation-wide ovarian cancer screening programme. Whilst I appreciate that a national programme needs a lot of evidence before using up a great deal of public money (it would be publically funded on the national health service the screening programme), I would argue strongly that for those women who individually want to safeguard their health, and their families' well being, the option of private screening should always be encouraged.

Which is why I wanted to know whether US is technically capable of detecting OC at such an early, and 'curable' stage. It only takes one 'positive' answer to 'prove' that - although I'm sure that factors such as the exact location of the early tumour, general abdominal factors, the quality of the US equipment and the skill and experience of the radiologist and oncologist, all play a role in making an accurate DX. The last thing anyone wants is a false negative!

I've actually had both a transvaginal US (about nine months ago) and an abdominal one (a couple of weeks ago), and neither found any cancer. The recent one found a two-inch cyst, apparently follicular, apparently fluid-filled, and probably only physiological and caused by my PCOS and entry into peri-menopause (confirmed by oestrogen/FSH levels taken recently). I'm now on 'watch and wait' to see if it clears on its own, as I understand many do.

I'm pretty sure I'm 'OK' in terms of OC BUT BUT BUT I think I'll still book to see a gynae, and, possibly, as you recommend (very sensibly!) a gynae onc. After all, every woman who is confirmed to NOT have OC saves not just herself, but precious medical resources for those who DO have OC.

But I'm reassured to know that US can, in principle, detect OC at such an early stage (though I appreciate that's probably via transvaginal, not surface.)

Thank you again for your encouraging reply, and all the very very best to yourself for your own treatment. You must be so relieved they found it so early, with such better prospects as a result.

Regards, Julie.

I was diagnosed with stage 1 in both ovaries and uterus in 2006.( I had transvaginal ultrasounds 1 year prior for cysts that would come and go.) I had been feeling bloated and tired. I developed constipation and abdominal pain. The pain had gotten to the point where I had to go to the emergency room. They did several tests, xray, etc and finally called in my Ob/gyn who ordered a contrast Ctscan and a CA 125 test. He gave me the news later that night that I had ovarian cancer. They never truly diagnose iovarian cancer until surgery. I was sent to Rush hospital in Chicago, IL where I had the surgery and was diagnosed. Luckily with surgery and 6 rounds of carboplaten/taxol I am cancer free 2 1/2 years later.

Hi,

I was recently diagnosed with stage one ovarian cancer and unfortunately two ultra sounds and an MRI did not pick up the cancer.  As a matter of fact Both the MRi and the ultrasound gave the impression that there was a mass on my left ovary.  All thought it was fluid, possibly a hemmatoma.Because it was large (6cm)  My gyn urged be to have it removed.  She removed it laparscopicly.  The surgery revealed that it was not a mass on my ovary but the ovary itself that was enlarged and it was 8cm not 6.  Because it was so large she drained it and then removed it.  Now I need to go through chemo  because it is considered a rupture.  I have since had staging surgery from and oncologist gyn.  There was no sign of the disease elsewhere thank God.  So my advice is to read the article featured on this site today and go to an oncologist if you have any ovarian cyst, mass or whatever.  Hop this helps.

I was diagnosed with STAGE IIIC only because the cancer had surrounded a lymph node in my groin and I could feel it and see it, albeit small. It is 3 years later and I just had a Normal CT Scan. But I wanted to answer this because I read one answer and the gal said that she was DX with Stage I and had a hysterectomy and chemo - I was under the impression that Stage I ovarian cancer when caught early more than likely did need to be treated with Chemo therapy. I would like to know more as early detection is key - but why treated with Chemo at Stage I plus a hyst?

Julie,

In response to your question about ultra sound and ovarian cancer..

I was diagnosed with Stage 3 ovarian cancer.  It was very difficult to find even at this stage.  I had an ultra sound and several CT scans which did not show any problems.  The cancer was not found until exploratory surgery.

My gyn/onco surgeon Dr. Jacob Rotmensch at Rush) removed both ovaries, uterus, and cervix. Since I was 54 and already post menopausal this was standard procedure. He also biopsed lymph nodes and the fatty tissue. 6 rounds of chemo is standard protocol in the US. OVCA sends out "clear cells" which can metastisize anywhere. Although the surgery and chemo were no picnic, I feel better than I have in a long time. I had the surgery and chemo in 2006.

Hi Julie:

Yes, an ultrasound can find Stage 1 ovarian cancer. It found mine. I was suffering from recurring urinary tract infections. When I had my third one in six months my wonderful doctor sent me for an ultrasound. A mass was found in my right ovary. She then sent me for an MRI with contrast. She then sent me to an onocological gynecologist for a second opinion. He was quite alarmed and scheduled me for laparoscopic surgery the following week and sent me for a CA125 blood test. The morning ot the surgery he came in and told me the results of my CA125 was 3,074 and that I defineitely had ovarian cancer and that he was going to open me up and remove the uterus and the ovaries and anything else he needed to remove. That was August 1005. My quarterly CA125's have as low as 1. Stage 1 has the best cure rate. It's tough but we make it through. Good luck and may God Bless you.
