Sutent For Mrcc

Pt in his early 50s, who had a radical nephrectomy 3 months ago after presenting with a large hematoma that proved to be RCC, now presents with 3 huge hematomas. One 19cm mass in the tumor bed, one in the pelvis, and a third smaller one in or adjacent to the vena cava. MRI shows a lot of vascularity and new blood vessels in these masses, which are presumed but not confirmed to be metastatic RCC. Right now there is discussion on whether or not to attempt to resect these hematomas, now that bleeding has tapered off, and Hgb is stable, or to begin Sutent or Sorafenib. Pfizer is very interested in this unusual presentation, and advocates Sutent, even without failing IL2 or other. Can anyone comment on use of Sorafenib, sutent, or AG-013736? Is there any in vitro assay that can determine which of these would be most effective? Thank you
