Stenosis after 2 Cones

Hi,

I was pregnant in 2007 when I got my first abnormal Pap. They did a few more and some colpolscopie's until I got a punch biopy after the birth. The punch revealed CINII so they went in to do a Cold knife Cone. The margins ended up being unclear and showed CINIII (carcinoma in situ) extending further up and down. So I had another Cone in May 2008 and the margins were clear, I have been going in every few months for rePaps.

The Paps have come out fine but they have not been able to get the endocervical cells due to a stenotic cervix. The Cones have left so much scarring that she it unable to get to those cells. She has tried to premedicate with Misoprositol to dilate and soften it prior to an office Pap but it has been very stubborn, not to mention how excruciating it is when she tries to get the cells.

For the past few months I have been extremely tired, my abdomen is huge (I look like I am in my 4th month of pregnancy), I am nauseous- and have to urinate often, I always feel full and my period has a labor-like pain. The symptoms are getting worse as time goes on.I must say that I am confident I am not pregnant, unfortunately there isn't even a chance, I have pain upon intercourse also. I am hoping that all these symptoms are from the small cervical canal not releasing as much menstrual fluid and not something serious. Menstrual fluid does pass over a period of a week and a half or so, but maybe some is staying.

Anyway, I have an appointment Monday morning in an Ambulatory center for a dilation and an ECC for biopsy. Does anyone else have stenosis or stricture of the cervix and will maintanence always include surgery to collect cell samples or does it get better? Is a hysterectomy an option if this continues?

I will post later in this thread with the results of my lab reports from Monday's procedure.

Any advice or stories are welcome. Thanks.

How thoroughly did your doctor discuss the risk of cervical stenosis with you? In addition, did your doctor tell you what symptoms - an inability to menstruate, extreme cramping when menstruating - to watch for? How you answer those questions will help you decide whether or not you should keep that doctor.

I do hope your appointment went well and that this is resolved in a good way for you.

My cervix is also badly scarred. But in my case, it was from a botched LEEP. If I were still planning to get pregnant, I'd need a doctor's help. It's that disfigured.

Thanks for your reply. :)

No, she didn't go over it with me in detail, it was probably on the consent form no doubt. I just didn't really "think" about what it would entail. Yeah, it's pretty awful. I had no idea, it really affects your quality of life.

The surgery went well, I think, my doctor didn't even talk to me or my husband afterwards. She told me prior to the surgery that there was a dimple there and either she was going to poke through it or leep out the tissue. So, who knows. I asked the ambulatory center what she did exactly, (if they had any surgical notes etc.) and they just said a dilation and ecc. Actually, they keep saying "dillitation", I swear, I can't believe I let these people near my vagina. :D

I'm still very tightly bloated and there is nothing significant coming out.

I called the doctor's office and talked to the nurse to find out- and she didn't have any surgical notes. I asked her if the trapped fluid will come out on it's own (like with my normal cycle) or would she have evacuated it, and she had no idea and told me she didn't understand the question. She said I was probably bloated from pain medication and call after the weekend.

From reading what I have on here, I think I should go to a gyno-oncolgist for the maintenance check ups. Don't you? Or at least switch doctors. she is leaving the practice in a few weeks anyway. (I don't know why, probably nothing crazy) I only kept going to her because she was the one who delivered my son (she was on call that day.) She is really nice and I am not a very forward person. I know I should be my own advocate, especially now, I am so uncomfortable.

I am guessing that the relief will come when my period is due. Hopefully, if not, I think I will request an ultrasound or something so they can see what is going on. And of course, I'm anxious about the lab results.

Fire your doctor and get a new one. This is your body, and she was required to inform you of the risks, in addition to the benefits. I don't care what is on the consent form. She was still required to talk to you. Most people are so frightened or are in a hurry and don't thoroughly read the information they are handed. Your doctor had to know that. She is also required to tell you exactly what she did. I am sorry she put you through this.

I definitely hope you'll be fine, now. But please don't go back to her. Your body is too precious and should only be trusted with a doctor who cares enough and is ethical enough to wanted you to be educated enough to make informed decisions.

Just in case someone is keeping up - my pathology report came back fine. I started getting relief when I got my period 7 days ago. It was a horrible period but I expected it, lots of cramps, large clots and pain. But, I have lost 6 pounds since then. My bloating is virtually gone and I feel so much better.

Hopefully my cervix will remain open. Thanks again.

I'm glad you're feeling better, but please get another doctor. She didn't take the time to educate you. This is against regulations, and she shouldn't be trusted.
