Top Hat Leep Results - need help on margin info!

My recent pap from May09 said "Carcinoma In Situ, rule out invasion."

Took 9 weeks for my GYN to contact me about results!

My Gyn scheduled me for a Top Hat Leep for August 4th.

Results are in and she wants me to come back in 4 months for another colposcopy and to scrap my endocervical canal? She thinks she cauterized everything, but it sounds like she wants to do some further procedure in the endocervical canal. She never mentioned that the margins were clear and I don't understand what these margins mean. I made sure to get a copy of my Leep result:

ENDOCX:

Severe Dysplasia CIS3 w/HPV31

Distal Margin Focally Involved

Cervix measuring 1.4 X 0.5 X 0.4

ECTOCX

Severe Dysplasia CIS3 w/HPV31

Proximal Margin Involved

Cervix measuring 1.3 X 1.1 X 0.4

 

I am scheduling an appt with my Family Physician for a second opinoin.

I am also scheduling an appt with an ONC/GYN as I want to make informed decisions. I really would like to just have a hysterectomy. I have 2 children. I am done with having kids. My hubby even had a vasectomy. I dont want to have to worry about this anymore.

 

Can anyone explain what these margins are?

K so I am going to explain Margines. When they do these procedures they take out a peice of tissue. The margines are the edges of this peice of tissue. When a pathologist looks at it under a microscope and sees that the abnormality is all the way to the edge of the tissue peice, it means that didn't get it all and that the margines are no clear. But if the pathologist looks at the tissue under the microscope and the abnormal cells end before the edge of the tissue, that the margins are clear.

 

9 weeks is far to long, but sometimes labs are behind in their work. So it does happen.

 

So from your pathology report I can tell that they did not get it all. So further test are needed to be done. If I were in your situation I would get a hysterectomy, but leave the ovaries in.
