How do lifestyle factors and exposure to environmental substances affect our cancer risk?

Has anyone else had difficulty getting a PET Scan approved by their insurance? My MD's have put in at least 3 requests and have been denied every time with ins. saying a CT scan was good enough. Seems suspicious as the radiologist always says the CT scan needs confirmation by a PET Scan.
