Have you discussed the fatigue and confusion with his doctor?  It's amazing that he is able to work at this time.

They might want to do another PET scan and brain MRI.

Confusion and fatigue could be lingering effects from the chemo or brain mets. 

Listen to yourself.  You know better than anyone what's going on with him.  Follow your gut. 

My heart and prayers go out to you.
