Rfa

Hi Kamal,

Incidentally my dads name is kamal too. My mom has been udergoing treatment for Colon cancer and has had a year of chemo after cleaning out the cancer...originally she was on oxaly and leucovorin etc but found mets in liver and so then she was on avastin with campto and xeloda from May thru Nov 2005 and for the past 3 months has been on no treatmnet. She had originally 2 liver lesions but the avastin etc did not help and the lesions multiplied. We were hoping to put her on a trial of Erbitux but she tested negative for it and so wih that our hopes were dashed.

Currently she has multiple lesions the largest of which is 2.5 cms x 1.6 cms. She has some nodal involvement, para aortic and coeliac group but her cancer is only in the liver.(about 10 oc of liver mass)

We consulted a specialist in RFA and he feels she is not a text book case as she has multiple small lesions but has agreed to go ahead with the procedure. The oncologist and this Dr both feel additional chemo (HIA) will only put additional strain on liver at this point. What do you feel are her chances with this treatment?? She is 70 yrs and in great spirits and her Quality of life is pretty reasonable- she is quite active and normal.

Thanks for your help.

Rfa

Dr. Morar:
I am scheduled for RFA in the coming week at Boswell Hospital in Sun City, AZ. I wonder if I should insist on chemotherapy after. My cancer is non-small cell lung cancer & has been found at Stage I thanks to my being a participant in UCLA's National Lung Screening Trials (NLST). Any information concerning further treatment will be greatly appreciated.

Questions on Carcanoid Tumors

I was initially dx with carcanoid tumors of the left lung(this was verified after 8 bouts of pneumonia). I had half of the left lung removed. After about 6 months I was symptom free (no flushing, loose bowels and other secondary effects from the carcanoids. After that time frame it was dx that I have carcanoid tumors in both the front and back of the liver. They did 4 radiation ablations and insertion of microscopic flashetts to hopefully cut the blood supply to the tumors. This was done with negative results. I have numerous tumors front and back of the liver and they are unable to do surgery because of the amounts. They stated that if it was either front or back alone then they could cut either section. I am underweight and just dx. with diabestes mellinitus type 2. The dr.'s state that because I was exposed to Agent Orange (dioxin)in Vietnam and the area I was in was sprayed very heavily. They gave me service connection for lung/liver and type 2 as directly related to the dioxon. I have just been accepted to Moffitt Cancer Hospital in Tampa commencing September 14th 2006.I am taking 3 shots of Sandosation and a monthly shot of LAR 30MG of Sandosation. Still have alot of flushing/very very loose bowels and they just palced me on Provogil 200MG to stay awake during the daytime hours. What other meds are available either as a direct med and or expermintal. How is interfuron with carcanoid issues? Thank you for any information that you may render. I fully understand that we are not a dr. patient relationship and therefore waive any legal issues associated with you information. Your information is strickly informational.Brian

Carcinoid

I can not coment on the medications, this is not my area of expertise. However, there is a very effective treatment for metastatic carcinoid lesions to the liver called Chemo-Embolization. This is a procedure that I do or can be done by many interventional radiologists. If you are near the Dayton Ohio area you can call me at 937-424-2580.

Rfa

My husband has stage iv lung cancer. We found out in April the cancer has spread to his liver. there are about 5 tumors in his liver and I was wondering if RFA could be an option for him. Please say that you can help him!! One other place the cancer is, is in the left adrenal gland. This tumor has remained stable for the last year but at our last visit this week there was no mention of this tumor. It was 2cm so mabye chemo took care of it??

Liver/adrenal Rfa

I have performed RFA's on both areas. However, the size and location of the lesions determine if I would be able to do the procedure. I need to see his CT scan. If you live close to Dayton Ohio, feel free to see me. Otherwise, you can send me his films by mail.
Liver Lesions

He has two in the lower lobe, one in the right lobe and one in the dome of the liver. the largest measured 5.5 cm and the other three were less than 2 cm. the lesion on his adrenal gland is about 2 cm. Has remained stable since October 2005. I know this doesn't give you much to work with; however, I will call your office for the address to mail the films.

I will be happy to help God Bless,Hello Dr.-My 46 yr old girlfriend has Stage IIIB OVA with a metastasis in a single lymph node of the celiac axis node, confirmed by PET/CT scan recently, she underwent Carbo/taxol chemo-6 sessions finishing 2 weeks ago. Her CA-125 is down to 10 from 45. Can RFA be done for the node as it is likely inoperable due to its location near the aorta? It had an SUV of 7.0 in March but we haven't got the Pet/CT results from a week ago yet. We have a well-known surgeon-gyn/oncol Dr. Michael Berman from UCI and GOG trials. Do you know him? We will be willing to try RFA if it makes sense to and if it can lead to a favorable outcome on getting rid of the lymph node involvement. She has medium differentiated cells, was "optimally debulked" and that was the only metastasis at the time. All of the lymph nodes in the abdomen around the peritoneal area came back negative. Dr. Berman also removed the omentum, and a couple of malignant nodes , one on the bladder and the top of the vagina with plenty of margin. He is less concerned with the abdomen than he is with the celiac lymph node. We are willing to travel and find no expense to be spared. If you can respond to this message it will be greatly apreciated.

RE: Cancer Ablation

RFA can be done, however the person doing the procedure would have to evaluate the imaging to see if it is possible. Lymph node ablations are not covered by insurance and long term outcomes are not always known. Other things you can look into for omental metestatic desease is intraperitoneal chemo therapy. The problem with metestatic deasease to the omentum is lack of good blood supply to carry the Chemo to the cancer. With intraperitoneal chemo, you may be able to increase the amount of chemo that gets to the targeted area. You can ask your oncologist about it. Dr. Morar
