Mass involving superior mesenteric vein and superior mesenteric artery. Inoperatable. Pancreatic cancer grade 3 of 4.   Two rounds of Gemazar/Abraxane given on (May 2 and May 16) and two more rounds recomended to shrink tumors before attempting IRE removal of tumor wrapped around viens.  Yesterday informed that tumor marker  rose from 2000 to 14000 after second round of Gemzar/Abraxane.  Should treatment be changed or continued for third round and scan?  Have others had this experience?

Hi. I have been diagnosed with stage 4 pancreatic on Dec 2017. I started my treatment with Folfirinox. After 3 consecutive increases in the marker (and significant deterioration of my physical condition) my oncologist switched the treatment to the Abraxane/Gemzar combo and that eventually reduced the marker value by more than 95%. A CT showed a substantial reduction of the tumoral masses as well. The opposite might be true for you (i.e., you may respond only to Folfirinox and not to Abraxane/Gemzar), so if your marker has not responded positively by now, it may be a good time to ask your oncologist about a switch to Folfirinox.
